How do I receive my test kit?

Health care providers order test kits by calling our laboratory, at (800-292-9019). You will pick up your kit at your physician's office, or Commonwealth Laboratories can ship the kit directly to your home.

Where will I take the test?

Some patients take the test at their physician's office. However, given the length of time involved, most patients prefer to take the test at home.

How do I return my test kit to your laboratory?

After placing the tubes containing your breath samples in the bubble wrapper, seal the carton and bring it to your post office. Postage is prepaid by Commonwealth Laboratories, Inc.

How soon will I receive my results?

The samples will be analyzed by our technicians with 2 business days on their arrival. Results will be faxed to your physician within one business day after testing. The physician will share results with you during your next appointment.

Will my insurance cover the cost of the test?

Most medical insurance providers cover the cost of the test. However, the patient is responsible for any deductible charge or co-pay assigned by his/her insurance plan.

Do I need a pre-authorization before taking the test?

This depends on your particular health plan. Even within the same medical health insurance company, some plans require pre-authorization and others do not. It is best to contact your health insurance representative at the number shown on your card, and inquire about pre-authorization. As very few insurers have in-network laboratories that provide this testing, you may need to have an out-of network referral. If you receive a pre-authorization number, please write it on the information form you will be sending to us with your breath samples.

For which insurers is Commonwealth Laboratories a participating provider?

We bill all major insurers. We are a participating provider with the following insurance companies:

  • Medicare
  • Blue Cross/Blue Shield
  • United Healthcare
  • Tricare
  • Harvard Pilgrim
  • Humana (Choice Care Network)
  • Coventry Healthcare of Iowa
  • Neighborhood Health Plan
  • Medicaid (Massachusetts, South Carolina, Iowa, Illinois)*
    *applications to become a Medicaid provider in other states are in progress

What if I don't have medical insurance?

If you have no medical insurance, please call our office for information. We work with patients individually to determine a payment plan that will be appropriate.

May I take more than one test on the same day?

No. Each test must be taken after fasting for 12 hours. Taking two tests on the same day will produce invalid results.

What if I have questions when I am at home taking the test?

Please call Commonwealth Laboratories with your questions. We are open Monday-Friday, except on major holidays, from 8:00 a.m. to 5:00 p.m. We will be happy to answer your questions. You may also e-mail us if you are taking the test on the weekend. We check our e-mail when the office is closed, and will respond as soon as possible to assist you.
